Student Transcript Request

If you are a student or former student of  Creekview High School, Newman Smith High School, Ranchview High School, or R.L. Turner High School, you may request a copy of your High School Transcript by downloading and completing and following form. Student Transcript Request.

The request, along with a copy of your Drivers License or a Photo ID, may be brought by in person or mailed to the Records Management Office, 1445 N. Perry Rd., Carrollton, TX 75006. Faxes or email will not be accepted. Please include $5.00 per transcript and allow a turn-around time of 1 -2 weeks.

If you are still in high school, you must visit the Records Department within your school to obtain a copy of your transcript.
