Student Transcript Request

If it has been more than one year since and you are a student or former student of  Creekview High School, Newman Smith High School, Ranchview High School, or R.L. Turner High School, you may request a copy of your High School Transcript by downloading and completing and following form. Student Transcript Request.

Cost:  $5.00 per transcript

Accept: Cash, Money Orders, or Checks. Make checks out to CFBISD

The Student Transcript Request, along with a copy of your Drivers License or a Photo ID, may be brought by in person or mailed to the following address:

Carrollton-Farmers Branch ISD

Carrollton, TX 75006

Faxes or email will not be accepted. For request through the mail, please allow a turn-around time of 2-3 days.

** If you are still in high school, you must visit the Records Department within your school to obtain a copy of your transcript.

** For students who just graduated,  you must contact your school to obtain a copy of your transcript.

 Hours: 8:00 a.m. to 4:30 p.m.
